Gastric Fibrolipoma Causing Bleeding in a Child

Abstract

Gastrointestinal lipomas are uncommon benign tumors usually occurring in the colon and rarely in the stomach. We report a case of a 10-year-old boy who presented with a two-week history of epigastric abdominal pain and several episodes of melena. Gastroscopy revealed a soft, elevated, broad based, polypoid lesion on the posterior wall, without superficial erosion or ulceration. One week later the patient was readmitted with melena and hematemesis, followed by a significant drop of hematocrit levels. A laparotomy was carried out and the mass was excised. Histological findings were consistent with a submucosal gastric fibrolipoma resected in toto. The clinical presentation, diagnosis and management of this condition are discussed.

Fibro-lipome gastrique responsable d'une hémorragie chez un enfant

Les lipomes gastro-intestinaux sont rares, survenant le plus souvent sur le colon et rarement sur l'estomac. Nous rapportons le cas d'un garçon de 10 ans qui s'est présenté avec des douleurs épigastriques et plusieurs épisodes de méléna évoluant depuis 2 semaines. La gastroscopie a révélé une lésion polypoïde, molle, élevée, à base large, sur le mur postérieur de l'estomac, sans érosion superficielle ou ulcération. Une semaine plus tard le patient a été réadmis avec un méléna et des hématémèses, suivis d'une chute significative de l'hématocrite. Une laparotomie a été réalisée et la masse excisée. L'étude histologique a montré un fibro-lipome gastrique sous-muqueux réséqué en totalité. La présentation clinique, le diagnostic et la prise en charge de cette lésion sont discutés.

Los lipomas gastrointestinales son tumores benignos raros que generalmente ocurren en el colon y rara vez en el estómago. Presentamos el caso de un niño de 10 años que se presentó con una historia de 2 semanas de dolor abdominal epigástrico con varios episodios de melena. La gastroscopia reveló una lesión polipoide blanda, elevada y de base amplia en la cara posterior sin erosión superficial o ulceración. Una semana más tarde el paciente volvió con melena y hematemesis seguida de una caída significativa del hematocrito. En la laparotomía se extirpó la masa que resultó ser histológicamente un fibrolipoma gástrico submucoso resecado en su totalidad. Discutimos la presentación clínica, el diagnóstico y el tratamiento.

Fibrolipom des Magens als Ursache einer Blutung bei einem Kind

Gastrointestinale Lipome sind sehr ungewöhnliche, gutartige Tumoren, die im Allgemeinen im Dickdarm und nur selten im Magen lokalisiert sind. Wir stellen einen 10 Jahre alten Knaben vor, der wegen seit zwei Wochen anhaltenden Bauchschmerzen mit Schüben von Meläna vorgestellt wurde. Bei der Gastroskopie fand sich eine weiche breitbasige polypoide Veränderung an der Magenhinterwand ohne Erosionen oder Ulzerationen. Eine Woche später wurde der Patient wegen Meläna, Bluterbrechen und einem deutlichen Hämatokritabfall wieder vorgestellt. Im Rahmen einer Laparotomie wurde der Tumor entfernt. Histologisch fand sich ein in toto reseziertes, von der Submukosa ausgehendes Fibrolipom.
